CHRONIC DISEASE

& HEALTH EQUITY

PROGRAM

The Chronic Disease and Health Equity Program at the Institute for the Advancement of Minority Health is dedicated to enhancing health outcomes for underserved and marginalized populations across Mississippi.

About us

ADVANCING HEALTH.

BRIDING EQUITY.

The Institute for the Advancement of Minority Health’s Chronic Disease and Health Equity Program is committed to improving chronic disease outcomes among underserved and marginalized communities in Mississippi.

Through strong partnerships, collaborations, community engagement, and education, the Institute provides culturally relevant information on chronic disease management and health access to those who need it most.

Via ongoing relationships with local coalitions and state/federal health agencies, the Institute provides its partners access to expert-led presentations, technical assistance, and strategic planning support. This approach fosters continuous learning, collaboration, and sustainable impact for the communities served.

KEY INITIATIVES IMPLEMENTED:

  • Treasure Your Health Wellness Event: Focuses on preventive care and education, offering a wide range of services and resources to community members.
  • Choppin’ It Up Barbershop Series: Brings health education directly to Black men in familiar, trusted community spaces.
  • Youth Health Rallies: Exposes and engages Mississippi youth in health education to promote positive health behaviors.
  • Clear the Smoke Campaign: Focuses on reducing tobacco-related disparities in high-burden communities through education and targeted outreach.

These initiatives reflect our commitment to meeting communities where they are—providing health interventions that are accessible, culturally relevant, and designed to create lasting change.

IAMH Tobacco Coalition Spotlight

At our June Coalition meeting, the Chronic Disease team took a moment to shine a spotlight on the outstanding work being carried out by our dedicated coalition members across Mississippi. Their commitment to improving health outcomes in our communities is both inspiring and impactful. This special recognition is our way of expressing heartfelt appreciation for the tireless efforts of our partners. Whether through innovative programs, community outreach, or policy advocacy, our coalition members are making meaningful strides in the fight against chronic disease. We are proud to showcase their successes and elevate the incredible contributions they are making every day to build a healthier Mississippi.
