The purpose of this pilot intervention is to improve maternal and child health outcomes in Mississippi through health education, outreach, and linkage to care utilizing a community health worker model. The target area for this program is 1) women (ages 15-44) and children (ages 0-6) in Chickasaw, Choctaw, Claiborne, Copiah, DeSoto, Greene Hinds, Issaquena, Jefferson Davis, Itawamba, Madison, Perry, Rankin, Sharkey, Simpson, Sunflower, Tunica, Union counties in MS; 2) the partners are the women and children that participate in our community outreach and education activities.
